Request a sickness certificate (fit note)
If you are off work for 7 days or less, you do not need to request this from us. You should complete a self-certification form (SC2) yourself which is available from your employer or you can download it from the HMRC website.
My employer is insisting that I have a sick note for less than 7 days off work
We have to charge £10 for this, and will need discussion first. Please fill out the fit note form book using this link. You will be contacted within 24 hours by a health care professional.
Sick note for more than 7 continuous days off

Request a sick note. You will be contacted within 24 hours by a health care professional to talk to you about your reason for missing work. Any sick note that is issued will be sent to you electronically via a text message, and no longer requires a GP signature.
