For medical problems / pharmacy appointments
Please fill out the "medical request" form. We will aim to consult with you on the same day if you submit your form before 3:30pm. Please submit earlier in the day if you think you may need a face to face appointment.
Any requests after this time, or during busy periods, will be viewed but not actioned until the next working day unless your problem is medically urgent.
For nurse appointments
Please fill out the "Admin Query" form in the "something else" section with the reason for your appointment, and we will get back in touch with a choice of appointment times.
